Opportunity for DC Health Care Professionals
Sorogi is exicted to announce a partnership with Flexcare Pharmacy and DC Health to provide training and technical assistance related to diabetes prevention and diabetes management in order to increase the number of organizations able to provide enhanced care for DC residents.

The Association of Diabetes Care & Education Specialists (ADCES) will be hosting six, 60 minute, live webinars presented by subject matter experts.  The trainings are designed to deliver measured learning outcomes consistent with Moore’s Levels 1-4.

ADCES is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), the American Academy of PA’s (AAPA), and the Commission on Dietetic Registration (CDR) to provide continuing education for the healthcare team.

ADCES is offering one (1.0) continuing education credit to webinar participants for each webinar as follows:  1.0 contact hour of nursing continuing professional development credit by the ANCC, 1.0 continuing professional education unit (CPEU) by the CDR, 1.0 contact hour by the ACPE for pharmacists and pharmacy technicians, 1.0 hour of AMA PRA Category 1 Credit by the AMA, and 1.0 AAPA Category 1 CME credit by the AAPA.

APhA | Delivering Medication Therapy Management Services

Program Overview

APhA’s Delivering Medication Management Services presents a systematic approach for developing, implementing, delivering, and sustaining MTM services.

It includes an overview of the marketplace for delivering MTM services, guidance for implementing MTM services in pharmacy practice, a review of the essential skills and knowledge needed for performing MTM successfully, and an organized process for identifying medication-related problems.  The purpose of this certificate training program is to prepare pharmacists to improve medication use through the delivery of MTM services in a variety of practice settings.

HOW IT WORKS

This program is conducted in three parts:

1)  Online interactive self-study with assessment

2)  Live Training Seminar with online evaluation

3)  Post-seminar case exercises

 

A Certificate of Achievement is awarded to participants who successfully complete all activity requirements:

  • Successful completion of the self-study component involves passing the self-study assessment with a grade of 70% or higher.

  • Successful completion of the live seminar component involves attending the full live seminar and completing the online evaluation.

  • Successful completion of the post seminar case exercise components involves completing 3 post cases and submitting an online attestation statement of completion.

CONTINUING EDUCATION

Successful completion of the entire program will result in 21 contact hours of CPE credit (2.10 CEUs).

  • 10 hours (1.0 CEU) is awarded for completion of the self-study learning portion | ACPE UAN: 0202-9999-18-164-H04-P

  • 8 hours (0.8 CEU) is awarded for completion of the live training seminar | ACPE UAN: 0202-9999-18-165-L04-P

  • 3 hours (0.3 CEU) is awarded for completion of the post seminar case exercise | ACPE UAN: 0202-9999-18-166-H04-P

    Once credit is claimed, Statements of Credit will be available online within 24 hours on participant’s CPE Monitor profile at www.nabp.net.  The Certificate of Achievement will be available online upon successful completion of the necessary activity requirements on the participant’s “My Training” page on www.pharmacist.com.

APhA | The Pharmacist and Patient-Centered Diabetes Care
June 2021

Program Overview

APhA’s The Pharmacist & Patient-Centered Diabetes Care Certificate Training Program is an intensive educational experience designed to equip pharmacists with the knowledge, skills, and confidence needed to provide effective, evidence-based diabetes care. Five self-study modules provide comprehensive instruction in current diabetes concepts and standards of care. The live seminar incorporates case studies and hands-on skills training focused on the situations most likely to be encountered—as well as the services most needed—in community and ambulatory care practice settings. Participants will gain experience evaluating and adjusting drug therapy regimens for patients with type 1 and type 2 diabetes, counseling patients about lifestyle interventions, analyzing and interpreting self-monitoring of blood glucose results, and assessing the overall health status of patients to identify needed monitoring and interventions.

HOW IT WORKS

This program is conducted in two parts: 

1)  Online interactive self-study with assessment

2)  Live Training Seminar

  • The live training seminar includes demonstrating competency in blood pressure testing, self-injection techniques, diabetic foot exam, and blood glucose testing, followed up with online evaluation and assessment.

 

A Certificate of Achievement is awarded to participants who successfully complete all activity requirements:

  • Successful completion of the self-study component involves completing all 15 videos and receiving a 70% or better on the self-study exam.
  • Successful completion of the live seminar component involves attending the full live seminar, completing a skills assessment, and passing the final assessment with a grade of 70% or higher. In addition, participants must complete a final program evaluation.

CONTINUING EDUCATION

Activity Type: Practiced Based

Target Audience: Pharmacists in all practice settings

Successful completion of the entire program will result in 23 contact hours of CPE credit (2.30 CEU).
