Job Title: Patient Care Advocate (Spanish Educator)

Job Description:

A Sorogi Patient Care Advocate (Spanish Educator) works with participants to improve their understanding of diabetes/hypertension, make lifestyle changes to improve their blood pressure management, provide support and accountability to attain desired health goals including medication adherence. The Health Coach will be required to but not limited to the following:

  •  Advocating for English and Spanish-speaking participants to ensure they get the best care and program satisfaction possible.
  • Presenting Sorogi programs to participants and confirming their understanding of who to contact with questions or concerns.
  • Bridging communication between participants and Sorogi staff by monitoring program participation and focusing on consistently improving participant engagement.
  • Performing administrative tasks, including answering and making phone calls, and text messages on sideline.
  • Onboarding participants onsite and virtually into Sorogi programs and training them on portal usage.
  • Weekly completion of participant data entry into excel workbook.
  • Collect and monitor health data from diabetes and hypertension program participants at various points throughout the day and shall electronically transmit secure data to participant’s healthcare providers or managed care organizations.
  • Alert the team on noticeable trends in data and any out-of-range results that may require further intervention from Sorogi team and/or the participant’s health care provider.
  • Scheduling participants visit with desired Sorogi program representative.
  • Prevents and resolves complaints by counselling with Sorogi staff who see a problem developing.
  • Offering suggestions to the team on how to resolve potential complaints and modifying practices that cause repeated issues.
  • Conducting and documenting monthly participant questionnaire and feedback.
  • Overseeing the daily program schedule and seeking opportunities to maximize scheduling efficiencies.
  • Participating in recruitment sessions and following up with potential participants.
  • Updating job knowledge by participating in continuous education sessions.
Job Title: Sorogi Office Manager

Job Description:

A Sorogi Office Manager maintains office services by organizing office operations and procedures, preparing payroll, controlling correspondence, reviewing, and approving supply requisitions, and assigning and monitoring clerical functions.

The Office Manager will be required to but not limited to the following:
• Work within the guidelines set forth by the company.
• Plan, manage, coordinate, implement and evaluate all company programs and initiatives to make sure they serve the objectives of the Company.

• Introduce Company programs to potential clients.
• Participate in on boarding processes for new and existing clients.
• Participate in weekly management and operation meetings.
• Evaluate, update, and streamline onboarding and training procedures.
• Work with management team to achieve financial objectives.
• Contribute to the development, implementation and evaluation of policies, procedures and guidelines related to the company quality assurance process.
• Work with management team to integrate federal, state, and local regulations and policies in all company activities as needed.
• Participate in staff recruitment and evaluation. • Documentation of all patient information in an accurate and timely manner.
• Participate in promotional events and health fairs.
• Attend trainings and workshops as required.

Job Title: Registered Dietitians

Job Description:

The Registered Dietitians will be required to but not limited to the following: Lead monthly education sessions for participants.

  • Work within the guidelines set forth by the Company
  • Assess members nutrition needs as it relates to preventing or managing chronic diseases especially diabetes, hypertension, and obesity.
  • Available for one-on-one and group appointments (virtual appointments)
  • Review patient health records before appointments
  • Provide education and support to members on their nutritional needs as it relates to preventing or managing chronic diseases such as diabetes.
  • Conduct group/ one-on-one education sessions on nutrition topics
  • Conduct monthly participant follow-up appointments as needed
  • Work with members to develop tailored, affordable, and culturally appropriate meal plans
  • Document and submit all session reports as per company policies
  • Work independently
  • Conduct initial evaluation all company nutrition education programs and initiatives to make sure they meet the needs of our members and company objectives
  • Contribute to the development, implementation and evaluation of policies, procedures and guidelines related to the company’s quality assurance process
  • Participate in strategic and CQI meetings as it relates to nutrition and support services
  • Participate in promotional programs and projects locally and nationwide as needed.

Qualifications for Dietitian:

  • Registered Dietitian (Licensed to practice in DC/MD/VA)
  • ≥ 3 years working experience with diverse population

Skill Set:

  • Excellent oral and verbal communication skills
  • Work independently

Job Description: Sorogi Health Coach

A Sorogi Health Coach works with participants to improve their understanding of hypertension, make lifestyle changes to improve their blood pressure management, provide support and accountability to attain desired health goals. The Health Coach will be required to but not limited to the following:ReWork within the guidelines set forth by the company for Health Coaches.

  • Lead monthly education sessions for participants.
  • Provide timely interventions to ensure adherence to program goals.
  • Work with participants to determine their challenges, identify obstacles, and create a plan to achieve a healthy blood pressure.
  • Keeping participants motivated and on track to overcome challenges by routinely checking in.
  • Assist in designing session teaching materials.
  • Work with participants to develop individualized plans based on their current health record and limitations.
  • Attend bi-weekly strategy and training meetings either in person or virtually.
  • Participate in recruitment activities as needed for community wellness programs.
  • Documentation of all patient information in an accurate and timely manner.
  • Participate in promotional events, poster presentations, and health fairs.

Qualifications for Health Coach:

  • Bachelor’s degree in Psychology, Registered or Licensed Nurse, or Registered Dietitian
  • Local or lives within driving distance of Washington, DC
  • Non-negotiable adherence to all pertinent ethical guidelines.

A Sorogi Nurse Health Coach is fluent in both English and Spanish. A Nurse Health Coach works with participants to improve their understanding of diabetes and hypertension to make lifestyle changes to improve their blood pressure management, provide support and accountability to attain desired health goals. The Nurse Health Coach will be required to but not limited to the following:

Responsibilities for Nurse Health Coach

  • Work within the guidelines set forth by the company for Nurse Health Coaches.
  • Lead monthly education sessions for participants both in English and Spanish.
  • Provide timely interventions to ensure adherence to program goals.
  • Work with participants to determine their challenges, identify obstacles, and create a plan to achieve a healthy A1C and blood pressure.
  • Keeping participants motivated and on track to overcome challenges by routinely checking in.
  • Assist in designing session teaching materials.
  • Work with participants to develop individualized plans based on their current health records and limitations.
  • Attend bi-weekly strategy and training meetings either in person or virtually.
  • Participate in recruitment activities as needed for community wellness programs. These recruitment activities include but are not limited to publications, social media content creation, webinars, health fairs as well as interviews of additional support staff for wellness programs.
  • Documentation of all patient information in an accurate and timely manner.

Qualifications for Nurse Health Coach:

    • Licensed Practical Nurse (LPN) or Registered Nurse (RN) **Preferred**
    • Local or lives within driving distance of Washington, DC.
    • Fluent in Spanish with the ability to confidently communicate health-related topics in verbal and written format (ex. Medical Spanish).

Job Description: Health IT Manager

As the Health IT Manager for Sorogi, you will be responsible for maintaining the technology infrastructure, and software applications necessary for the day-to-day secure delivery of all health programs and operational support. You will also be responsible for implementing technology strategies and ensuring that allocated

resources are aligned with the company’s business needs. Duties also include reviewing all software applications,

implementing system upgrades, training personnel and external stakeholders on appropriate use of systems and making recommendations to the Board of Directors on future technology plans.

Responsibilities for Health IT Manager

  • Participate in the design and implementation of required IT roadmap to support all health programs, financial, and strategic missions of Sorogi.
  • Periodic evaluation of new software and hardware for usefulness and compatibility with internal and external workflow
  • Review gaps in current Sorogi Technology solutions (ex. Sorogi Health web app) and implement solutions that could mitigate challenges posed by these gaps
  • Assist in developing tools for data visualization as it pertains to Remote Patient Monitoring via Bluetooth-enabled devices
  • Provide leadership for tactical and strategic planning for all network-related activities, negotiate contracts, and integrate multiple vendors that provide technology-related services
  • Monitor KPIs and IT budgets to assess technological performance
  • Monitor system utilization and provide recommendations for capacity planning for servers, storage, and devices used.
  • Participate in local and national initiatives that advance the technology-enabled objectives of the company as needed.
  • Recommend technological tools that will improve participant adherence and engagement in programs and services.
  • Work with Sorogi team to implement appropriate internal and external technology training tools (materials, webinars, demos, etc.) to improve business operations
  • Analyze, evaluate, and propose network, security, telecommunications, storage, database, and technology architecture: hardware, software, protocol, and strategy
  • Research, recommend and implement technology tools that yield competitive advantage

Qualifications for IT Manager

  • Bachelors in Mechanical or Computer Engineering (graduate-level degree preferred)
  • Experience with Remote Patient Monitoring via Bluetooth-enabled devices
  • Very strong hands-on experience in software development and coding
  • Confident in health systems and devices as it pertains to cloud storage and systems integration
  • Expert in Office 365 tools, Google and Go Daddy website systems, hosting and backend analytics
  • Managerial experience in the technology field
  • Excellent verbal and written communication skills
  • Ability to understand multiple types of financial and legal documents
  • Commitment to creating and sustaining high-quality healthcare delivery
