We are Working with DC Health
Sorogi is exicted to announce a partnership with DC Health’s Chronic Disease Division to provide training and technical assistance related to diabetes prevention and diabetes management in order to increase the number of organizations able to provide enhanced care for DC residents.
Through this grant’s funding, Sorogi will provide training, technical assistance, and project management support to community pharmacies to achieve accreditation for their DSMES programs.
These trainings are offered at NO COST to DC Pharmacies.
DC Health Aims to Achieve the Following Goals
Build Capacity and Enhance Training
Build capacity and enhance training and technical assistance for organizations in the District implementing pre-diabetes and diabetes management and prevention support services.
Increase Engagement
Increase engagement of pharmacies in the provision of medication management or Diabetes Self-Management Education and Support Program (DSMES) for people with diabetes.
Improve Access and Participation
Improve access to and participation in ADA-recognized/ADCES-accredited DSMES programs in underserved areas.
