Combating Diabetes Health Disparities in Washington DC

The solution to health disparities lies in a collective community effort.  Sorogi is excited to support the Make A Difference pharmacy ambassadors to improve diabetes management for DC residents.  The Make a Difference program developed by Sorogi aims to increase access to diabetes resources in vulnerable communities in Washington, DC by engaging pharmacies to become accredited providers of Diabetes Self-Management Education and Support (DSMES) services. By leveraging the relationship and trust pharmacists have cultivated over the years within the community, the program ensures DC residents living with diabetes will be better equipped and empowered to take control of their health. Through a rigorous selection process, Sorogi was able to select four independent pharmacies to become providers of DSMES and inaugural ambassadors of the Make a Difference program.
