Dr. Vivian Nnacho Ayuk, PharmD, CDCES
Training Facilitator
Dr. Vivian Ayuk is an Alumna of the University of Minnesota College of Pharmacy with over 16 years of practice experience. As a clinical pharmacist and Certified Diabetes Care and Education Specialist, she has led multiple programs and pilot studies incorporating remote monitoring in personalizing care for patients with chronic disease states.
She works with a multidisciplinary team of professionals providing continuing education to healthcare providers to improve quality of care. Dr. Ayuk provides consultation services to healthcare organizations implementing community health initiatives that address barriers to care.
In additon to her work in the community, Dr. Ayuk is an Adjunct Clinical Faculty at Notre Dame of Maryland School of Pharmacy, member of the Association of Diabetes Care and Education Specialists Technology Committee, Community Pharmacy Enhanced Network, and Washington DC Pharmacy Association.

Provider Training
Chronic care management training is provided by Sorogi for healthcare and other service providers to help equip them with evidence-based skills to treat and care for patients with chronic disease conditions like diabetes, hypertension, cardiovascular disease, and depression. Training is geared toward clinical staff including physicians, physician assistants, nurse practitioners, nurses, medical assistants, and care coordinators.
