Diabetes Prevention Program

About Diabetes Prevention
Individuals at high-risk for Type 2 diabetes can prevent or delay the disease by losing a moderate amount of weight through lifestyle changes (dietary changes and increased physical activity).  This program is recognized by the Centers for Disease Control and Prevention (CDC).

Taking prescribed medications, may also help prevent or delay the disease.

 

Sorogi’s Diabetes Prevention Program consists of 25 small group classes facilitated by a Sorogi lifestyle coach who leads discussions of lifestyle change that can alter behavior and result in improved health.  The twenty-five one hour sessions are held over the course of a year.  

We are currently enrolling participants for our Diabetes Prevention Program.  These classes are covered by insurance and through a grant provided by the DC Department of Health.  All classes are currently being held remotely due to COVID-19.  When safe to do so, we will offer the option of in-person classes.

Thursday Evenings from 6pm – 7pm

Classes resume in October 2023.

What you will learn in Sorogi’s Diabetes Prevention Program

You will learn to make healthier meals by including lots of vegetables, watching portion sizes, and reducing calories.

During the first half of the program, you will learn to: 

  • Eat healthy without giving up all the foods you love
  • Add physical activity to your life, even if you don’t think you have time
  • Deal with stress
  • Cope with challenges that can derail your hard work – like how to choose healthy food when eating out
  • Get back on track if you stray from your plan – because everyone slips now and then. 

In the second half of the program you will build on the skills you have learned so you can maintain the changes you have made.

These classes will review key skills such as:

  • Tracking your food intake and physical activity
  • Setting Goals
  • Staying Motivated
  • Overcoming barriers

You may learn some new information too.  The lifestyle coach and small group will continue to support you throughout your wellness journey

Diabetes Self-Management Education Classes are led by a Certified Diabetes Educator and meet on Wednesday evenings from 6pm to 7pm EST.  All classes are currently held virtually via Zoom.
