DC Department of Health Chronic Disease Division
An ongoing partnership since 2020
We work with the DC Department of Health Chronic Disease Division to implement evidence-based self-management and prevention programs in medically underserved communities, promoting equitable care and early detection of diabetes and hypertension.
This includes the Pharmacy Wellness Hub Project, which supports community pharmacies with nationally accredited Diabetes Self-Management Education and Support (DSMES) programs. The project aims to improve health equity and outcomes and encourage the adoption of technologies such as Continuous Glucose Monitoring (CGM) and Remote Patient Monitoring (RPM).
African American Health Program (AAHP), Montgomery County, Maryland
An ongoing partnership since 2022
We work with AAHP to deliver targeted education and support services to prevent and manage chronic health conditions such as diabetes and cardiovascular diseases. Our partnership aims to equip a traditionally underserved community to successfully adopt healthy lifestyle behaviors.
Diabetes Demonstration Project
A proud partnership from 2022 to 2023
We collaborated with three DC MCOs (AmeriHealth Caritas DC, CareFirst Community Health Plan DC, and MedStar Family Choice DC) to build the infrastructure for a demonstration project that informed DC Health's efforts to achieve Medicaid coverage for DSMES services.
DC Capacity Building Project
A proud partnership from 2021 to 2024
We provided training and technical assistance to enhance the capacity of district healthcare providers to deliver diabetes management and prevention support services to adult Medicaid recipients.
Association of Diabetes Care and Education Specialists (ADCES)
An ongoing partnership since 2021
We provide comprehensive CGM live training to DC community pharmacists and targeted continuing education programs for all members of their care teams.
Diabetes Prevention Program
A proud partnership from 2020–2022
Sorogi received funding to implement the CDC-recommended, evidence-based Diabetes Prevention Program for individuals at risk for or with chronic conditions, with a special emphasis on women, low-income residents, African American residents, and Ward 7 and 8 residents.
Central Union Mission
An ongoing partnership since 2024
We work with the Central Union Mission to offer our programs to at-risk populations they serve, including unhoused individuals, families in poverty, seniors, and the growing Hispanic community in DC.
