Healthcare and Service Provider Training
Chronic care management training is provided by Sorogi for healthcare and other service providers to help equip them with evidence-based skills to treat and care for patients with chronic disease conditions like diabetes, hypertension, cardiovascular disease, and depression. Training is geared toward clinical staff including physicians, physician assistants, nurse practitioners, nurses, medical assistants, and care coordinators.

DCPCA has partnered with Vivian N. Ayuk, PharmD, CDE and Sorogi to develop and deliver customized training to our member health centers in order to help bridge the gap between the clinical and self-management aspects of care and address key topics for chronic disease management.
Over the past year, Dr. Ayuk has carried out numerous relevant and meaningful training tops which include Patient Engagement and Medication Adherence, Diabetes Medication Management, Understanding Diabetes Complications, and Gestational Diabetes. She has done an outstanding job in engaging health centers staff and providing a didactic and experiential learning environment.
Training Facilitator
Dr. Vivian Nnacho Ayuk, PharmD, CDCES
She works with a multidisciplinary team of professionals providing continuing education to healthcare providers to improve quality of care. Dr. Ayuk provides consultation services to healthcare organizations implementing community health initiatives that address barriers to care.
In additon to her work in the community, Dr. Ayuk is an Adjunct Clinical Faculty at Notre Dame of Maryland School of Pharmacy, member of the Association of Diabetes Care and Education Specialists Technology Committee, Community Pharmacy Enhanced Network, and Washington DC Pharmacy Association.
